If you are applying to DO schools, you already know that secondary applications are where the real writing begins. Your AACOMAS personal statement opens the door. Secondaries determine whether you walk through it.
The problem is that free, DO-specific secondary essay guidance barely exists. The big consulting firms paywall their examples. Reddit threads contradict each other. And the stakes are high: a weak secondary response at a school where you are otherwise competitive can quietly end your candidacy before you ever get an interview invite.

How DO Secondaries Differ from MD Secondaries
Before we get school-specific, you need to understand what makes DO secondaries distinct as a category.
MD secondaries (through AMCAS) tend to ask broad questions: why this school, describe a challenge, tell us about diversity, discuss a research experience. DO secondaries ask many of the same things, but with one consistent additional layer: why osteopathic medicine specifically.
This is not a small difference. At roughly half of all DO programs, you will encounter at least one prompt that explicitly asks about your commitment to osteopathic principles, your exposure to DOs, or your understanding of OMM. Some schools make it their primary essay question. Others weave it into a "why our school" prompt. Either way, you cannot escape it.
The implication is practical. If you are a dual MD/DO applicant and you plan to lightly edit your MD secondary answers for DO schools, you will get caught. Admissions committees at osteopathic programs are specifically trained to spot applicants who treat their school as a backup plan. The fastest way to signal "backup" is to submit a secondary that never once mentions osteopathic medicine.
Three rules that apply across every DO secondary below:
- Every "why this school" answer must include something specific to osteopathic education, not just the school's location or ranking.

What They Are Really Asking:
ATSU-KCOM is the founding school of osteopathic medicine. That heritage is not decorative -- it is central to their institutional identity. When they ask about your "experiences with osteopathic medicine," they want specifics: shadowing hours with DOs, OMM observations, conversations with osteopathic physicians that changed how you think about patient care. Vague enthusiasm will not cut it at the birthplace of the profession.
The "life experiences" prompt is broader, but they are still reading it through an osteopathic lens. They want to see that your personal story aligns with the values Andrew Taylor Still built the profession around: treating the whole person, understanding the body's capacity for self-healing, and serving communities that need physicians most.
Example Passage (for the osteopathic experience prompt):

What They Are Really Asking:
SOMA has a unique model: students complete much of their clinical training at community health centers across the country. This is not a standard rotation structure. You need to demonstrate that you understand this model and are genuinely excited about it, not just willing to tolerate it.
The CHC prompts are the most distinctive. If you have CHC or free clinic experience, lead with specifics. If you do not, be honest about that, but explain what draws you to that care model and how you have prepared yourself through related experiences (volunteering at underserved clinics, global health work, or community outreach).

What They Are Really Asking:
AZCOM shares its parent university with CCOM (below), and the prompts are nearly identical. The "why AZCOM" and "why should we accept you" questions are doing double duty: they want school-specific knowledge and a self-assessment in the same breath. At 1,500 characters (roughly 250 words), you cannot waste space on generic statements.
The yes/no questions about DO experience and OMM observation are deceptively important. If you answer "no" to both, your essays need to work harder to explain your commitment to osteopathic medicine.

The "why should we accept you" prompt is your pitch. Do not be humble to the point of vagueness. State what you bring -- clinical experience, research, life perspective, community ties -- and connect each asset to something CCOM specifically offers.

What They Are Really Asking:
DMU does not send a separate secondary. Their supplemental questions live inside the AACOMAS application itself. Do not treat them as afterthoughts just because they share a page with your primary application.
The main prompt packs three things into one question: your osteopathic exposure, its impact on you, and how it connects to DMU's mission. That mission emphasizes primary care and service to rural and underserved communities, particularly in the Midwest. If your osteopathic experiences were in a suburban dermatology office, you will need to work harder to draw the connection.
The second prompt is only 500 characters (approximately 80 words). This is not the place for a paragraph. List what you are doing: continuing clinical volunteering, completing a post-bacc course, working as an MA, conducting research. Be specific and current.

What They Are Really Asking:
KCU's prompts are more behavioral than many DO secondaries. They are not explicitly asking "why osteopathic medicine" -- instead, they are evaluating whether you have the resilience, empathy, and clinical awareness that their program demands. But make no mistake: your clinical experience prompt should reference a DO or osteopathic setting if at all possible.
The "someone in need" prompt is distinctive. KCU is testing your observational skills and your instinct to act. They want to see that you notice when people are struggling and that you do something about it -- not just feel something about it.
Example Passage (for the "someone in need" prompt):
During my second year as a peer tutor in organic chemistry, I noticed that one of my regular students had stopped asking questions. She still came to sessions but sat in the back, took no notes, and left early. I could have assumed she had it figured out. What They Are Really Asking:
NYITCOM's prompts are concise (1,000 characters is roughly 160 words), but they cover a lot of ground. The campus choice question is partly logistical and partly philosophical -- they want to know you understand the difference between their Long Island campus and their Arkansas site, which serve very different patient populations and communities.
The "community where you see yourself practicing" prompt is deceptively important. This is NYITCOM testing whether your career goals align with their training. If you write about wanting to practice in a major academic medical center, that may not resonate with a school that emphasizes community-based medicine and primary care.
Example Passage (for the "community where you see yourself practicing"):
I see myself practicing in a mid-sized community where I can be a consistent presence in my patients' lives over years, not just episodes. Growing up in a Long Island suburb, our family doctor was a DO who delivered both my brother and me, managed my grandmother's diabetes, and saw my father through a cancer diagnosis. What They Are Really Asking:
PCOM's main prompt is deceptively simple. "One aspect" means they want focus, not a survey of everything you like about the school. Pick one thing -- a specific program, a research center, a community partnership, a clinical training site, an element of their curriculum -- and go deep on why it resonates with who you are.
PCOM is one of the largest and oldest DO schools in the country. They know applicants are attracted to their name recognition. What they want to see is that you have looked past the brand and found something specific that connects to your values and goals.
The optional deficiency prompt is genuinely optional. Use it only if there is something concrete to address: a semester of poor grades, a gap year you have not explained elsewhere, or a withdrawal. Do not use it to re-pitch your candidacy.

What They Are Really Asking:
Touro is actually three distinct campuses with different prompts, and treating them interchangeably is a common mistake. The Harlem campus prompt is the most open-ended and the most philosophically grounded -- it explicitly asks you to connect your story to their mission. The California campus digs deeper into osteopathic tenets and personal characteristics. The Nevada campus is more factual and activity-focused.
For Harlem specifically, the mission is rooted in serving diverse and underserved communities in one of the most historically significant neighborhoods in America. If you write a generic "why DO" response that could apply to any school, you are missing the point.

WVSOM asks more questions than almost any other DO secondary, but each one is capped at 500 characters (roughly 80 words). This is not a school that wants philosophical essays. They want concise, direct answers that reveal your character quickly.
The rural experience question is not throwaway. WVSOM is located in a town of approximately 4,000 people in the Appalachian region. Their mission is explicitly focused on producing rural primary care physicians. If you have no rural experience, be honest -- but explain your willingness and interest.
The ethics question in 500 characters is a writing challenge. You need to name the situation, state what you did, and show what you learned, all in about four sentences.
